If you have become disabled, then you may be able to qualify for social security disability (SSD) until you are able to begin working again. To apply for SSD, you must have the following information:
- Social Security Number
- Proof of Your Age
- Medical Records
- Contact Information for Your Doctors and Hospitals
- Lab or Test Results for Your Disability
- Most Recent W-2 or Federal Tax Return
- Job Description and Company
In addition to this personal information, you must also provide information about your family members, should you wish to apply for benefits for them as well. This includes their social security numbers, proof of their age, and proof of marriage (for your spouse). Please note that any documents need to be originals or certified copies.
Once you have all this information, you need to complete SA-3368 and the Disability Benefit Application. Make sure that you have everything by reviewing the Disability Benefits Checklist . After you have applied, it may take up to 5 months for the government to process your claim.
